For MS Word, you may type directly on the form and print it out.
For PDF, you will need Adobe Acrobat Reader to open the form and print it out. For those with Adobe Acrobat Pro, you can type and print directly like MS Word.
Purchase orders are accepted.
For shipping overseas, please pay a total of $12.50 for shipping and handling for one book. Please add $1.25 for each additional book.
For shipping to Hawaii or Alaska, please pay a total of $7.95 for shipping and handling for one book. Please add $1.25 for each additional book.
To Pay by Credit Card
1. Go to “About the Books” page.
2. Click the “Order Online” button for the book(s) you want.
3. You will be forwarded to the Square Up page.
— Square Up is a secured online payment processor similar to PayPal.
4. Chose the quantity desired and click “Add to Cart.”
5. Click “Continue to cart” on the small pop-up window to view your shopping cart.
— You can also click on the cart icon in the top right corner.
6. If you need to add more books, click “Add more items.” Otherwise, skip this step.
7. If you are ready to checkout, click “Continue to payment.”
8. Fill out your information to finish the checkout process with Square.
9. Remember to click “Place order” to submit your order.
